Ferulic acid: the serum beat placebo — but what was tested was not ferulic acid

In two randomised, double-blind, placebo-controlled trials, the side treated with a vitamin C, E and ferulic acid serum did significantly better than the placebo-serum side on wrinkles and elasticity (at 12 weeks, 23.9% versus 6.8%, P < .001). The designs are among the best in this layer. But what was tested was a serum containing three ingredients, and both sides of the face had a procedure first.

Ferulic acid is a phenolic acid found in the cell walls of plants such as rice bran, wheat bran and coffee beans. It became well known in cosmetics not on its own but as part of a combination with vitamins C and E. The rationale for that combination is that vitamin C lasts longer when the three are formulated together.

Which puts this page in an awkward position from the start.

Every human trial of ferulic acid we found on PubMed tested that three-ingredient serum. We found no trial applying ferulic acid on its own. So the results below must not be read as 'the effect of ferulic acid'.

And yet those trials are among the best-designed in this layer: randomised, double-blind, comparing two sides of the same person, with a placebo serum as the control. On this site, only PRP and IPL have had conditions like that.

A good design and an unanswerable question sit on the same page. Below we separate them.

What is established

The same ingredient at a different strength, in a different formula, is a different product. These are the conditions the studies used.

It beat a placebo serum twice

Two trials by the same research group, with near-identical designs.

First, a 2026 facial trial. A 12-week, prospective, randomised, double-blinded, split-face study in 31 adults with signs of facial ageing.

  • Both sides of the face had microneedling at baseline and week 4
  • After that, one side received the antioxidant serum (vitamin C, E and ferulic acid) and the other a placebo serum, daily
  • Assessed every four weeks over 12 weeks

At week 12:

  • Modified Griffith's scale: 23.9% versus 6.8% improvement (P < .001)
  • Hemi-MASI (pigment): 31.2% versus 5.1% improvement (P < .001)
  • Skin elasticity: 39.0% versus 6.8% improvement (P < .001)
  • Marked or near-total improvement on the Global Aesthetic Improvement Scale: 89.3% of antioxidant sides against 7.1% of placebo sides

Second, a 2025 neck trial. A prospective, randomised, double-blind, split-neck study in 31 participants aged 30 to 65 with visible neck ageing.

  • Both sides of the neck had fractional microneedle radiofrequency twice, four weeks apart
  • Immediately after treatment, one side received the antioxidant serum and the other placebo, daily

At week 12:

  • Wrinkle severity: down 29.9% versus 18.0% (P < .001)
  • Elasticity: up 12.9% versus 2.3% (P < .001)
  • Global aesthetic improvement: 87.5% versus 14.3%
  • Histology: increased elastin, reduced senescence markers (p16 and γ-H2A.X)

Why these designs are good.

Comparing two sides of one person controls automatically for age, sun exposure and habits. And the comparator was a placebo serum, not another product — the 'product A versus product B' problem we keep flagging in this site's procedures layer does not arise here. One side had the ingredients and the other did not.

Both trials also took biopsies, meaning they looked at what changed inside the skin, not only at surface scores.

So why is this not 'strong evidence'?

The same reason given on the cysteamine page. Both trials had 31 participants, and the same research group ran them. The author lists overlap substantially. Two independent teams each confirming something carries different weight from one team doing it twice in two body areas.

We found no trial applying ferulic acid on its own

What the two trials used was a serum containing vitamin C, vitamin E and ferulic acid together. Three ingredients in one bottle, and that bottle went on one side of the face.

So the question those trials answered is 'does that serum work'. Not 'does ferulic acid work'.

Separating the two would take one more trial: a vitamin C and E serum against a vitamin C, E and ferulic serum. That would show the difference between leaving ferulic acid out and putting it in. The kojic acid page on this site has an example of exactly that design — a 1999 trial that put the same gel on both sides of the face and added kojic acid to one.

Ferulic acid has no such trial. We did not find one.

The commonly cited rationale is that ferulic acid stabilises vitamin C so it lasts longer. That claim is about formulation stability, and its evidence is laboratory measurement rather than an effect on human skin. We have not verified that laboratory data at source, so we have not entered it as evidence on this page.

What can honestly be written stops here:

'What if you just apply it' is unanswered

If the previous item was about the ingredient, this one is about the conditions.

In both trials, before the serum went on, both sides of the face (or neck) had a procedure.

  • 2026 trial: microneedling, twice, on both sides of the face
  • 2025 trial: fractional microneedle radiofrequency, twice, on both sides of the neck

Both procedures work by opening microscopic channels in the skin. A serum applied while those channels are open and a serum applied onto an intact stratum corneum do not get into the skin in the same quantity.

So the question these trials answered is, more precisely: 'applied right after a procedure, does this serum do better than placebo?' The answer is yes.

The question they did not answer is: 'what happens if you apply this serum daily with no procedure at all?'

The exosome page on this site records a problem of the same shape. There, both sides got the device, so the contribution of the material laid on top could not be separated at all. Here the situation is somewhat better — only one side had the ingredients, so the serum's contribution is separated. The condition attached is that it is a contribution measured on skin whose channels were open.

The practical implication: if you are planning to have a procedure, the results above apply to your situation directly. If you are thinking of using a serum with no procedure planned, those numbers did not come from your situation.

And finally — both trials had 31 participants over 12 weeks.

How to use it

  • The trials applied it daily over 12 weeks.
  • Concentrations were not reported. Both trials named the serum's constituents without stating how much of each. We do not invent numbers.
  • The results above came from use right after a procedure. With no procedure planned, the conditions differ.
  • The other two axes of this combination are vitamin C and tocopherol. Read those pages too.
  • Antioxidants do not replace sunscreen. The firmest evidence on this site is still over there.

Cautions

  • We have not verified the range and frequency of adverse events reported in the two trials above.
  • Applying it to skin whose channels have been opened by a procedure took place in a clinical setting. Do not try to recreate those conditions at home.
  • Ferulic acid browns as it oxidises — the same phenomenon as a vitamin C serum turning yellow. A product that has changed colour is better not used.
  • We found no established data on use during pregnancy.
  • Both trials had 31 participants over 12 weeks and were run by the same research group.

With a new ingredient, apply a small amount somewhere inconspicuous, such as the inner arm, for two days first. Stop if it reddens or stings.
